Index funds: Are mutual funds and ETFs that folllow an Index using a computer algorithm. 97% of ETFs are Index funds.
Actively managed funds: Are mutual funds and ETFs managed by a team of humans, and have higher expense ratio fees. ARKK ETF is an actively managed fund.
You bring up some very good points here. I think it might have been simpler to just compare Mutual Funds to ETFs as Index Funds are just a type of either that follow an index (i.e. S&P 500, Russell 1000, etc.). I would have added that some mutual funds may be difficult to get or more expensive to trade if you are investing outside of a retirement plan such as a 401k.

I never knew fidelity had those zero funds, I don't want to pay fees so I avoid etfs, index funds, and mutual funds. We would still get stuck with any taxes if they sell stocks in the funds for a profit though right?
